43SixtyAZ & Alliance Fastpitch Waiver & Team Roster
By participating in any 43SixtyAZ and Alliance Fastpitch event or tournament, the undersigned acknowledges, appreciates, and agrees to the following:
1.
Assumption of Risk: I understand that the risk of injury and/or illness from participation in this program is significant. These risks include, but are not limited to, the potential for permanent paralysis, illness, or death. While rules, equipment, and personal discipline may reduce these risks, they still exist.
2.
Exposure to Communicable Diseases: I recognize that participation may involve contact with individuals who have been exposed to or diagnosed with communicable diseases, including but not limited to COVID-19 or other medical conditions. I understand that it is impossible to eliminate the risk of exposure or infection.
3.
Voluntary Participation: I knowingly and freely assume all such risks—both known and unknown—even if arising from the negligence of the "Releasees," and I take full responsibility for my participation.
4.
Compliance with Rules: I agree to follow all stated and customary terms and conditions for participation. If I observe any significant hazard during my presence or participation, I will immediately remove myself from participation and notify the nearest official.
5.
Release of Liability: On behalf of myself, my heirs, assigns, personal representatives, and next of kin, I hereby release and hold harmless 43SixtyAZ, Alliance Fastpitch, their employees, staff, umpires, participating coaches, and the City of Phoenix; including their officers, officials, agents, sponsors, advertisers, and—if applicable—the owners and lessors of the event premises (collectively, the "Releasees") from any and all claims for injury, illness, disability, death, or loss/damage to person or property, whether arising from the negligence of the Releasees or otherwise.

Acknowledgement of Terms: I have read this Release of Liability and Assumption of Risk Agreement, understand its terms, and acknowledge that I am giving up substantial rights by agreeing to it, either on my own behalf or on behalf of a youth participant associated with this guardian account. I sign it voluntarily and with full understanding of its implications.
By signing below, I agree and verify the following:

I consent to and assume the risks associated with participation in these programs.

I expressly agree to the terms of the release as outlined above, and I release and indemnify the Releasees from all liabilities related to my participation, even if arising from their negligence.
